2. Information collected
2.1 To undertake an assessment, I require basic information such as name, date of birth and relevant school/workplace/ home address details. The date of birth is required to allow for standardised tests to be scored, as a candidate's exact age has to be worked out (in some cases, depending on the test battery used). Address information is required to complete the report's cover sheet.
2.2 Background information will be gathered from the child/ parent and school as appropriate, and occasionally copies of relevant reports may be provided to inform the assessment. This may include sensitive medical information where applicable. Only relevant information will be included in reports, although the information will be written down to support the assessor in determining the diagnosis.
3. Sending Information
3.1 Reports are communicated electronically as a password-protected PDF
3.2 Background information is requested by email at the point of booking services; you are under no obligation to include any identifying details in this document. However, it is preferable to have basic information of previous diagnosis and other information as outlined on the form in advance to allow for the assessment to be planned to meet specific needs. This will be sent as an editable Word document.
3.3 Draft reports are confidentially shared for proofreading with Kathryn Churchill from Executive Solutions Ltd.
4. Storage of information
4.1 According to UK law, assessment reports will be kept securely for six years after a child reaches their 18th birthday. Reports will be stored on a password-protected computer and encrypted cloud storage.
4.2 After the assessment and compilation of the final assessment report, all client questionnaires, raw data test sheets, and other personal data will be permanently deleted/destroyed.
This policy was created in August 2021, reviewed in August 2023 and will be reviewed annually.
